7. WHAT INFORMATION DO I NEED TO BRING WITH ME FOR HOSPITAL ADMISSION?

Patients should bring the following information:

·         Health insurance ID or personal ATM debit card

8. WHAT ARRANGEMENTS DO I NEED TO MAKE WITH MY HMO PRIOR TO TREATMENT ?

Individual HMO programs vary. Please contact your HMO carrier regarding questions that you have with your plan. Many insurance companies require prior authorization for surgical procedures, hospital admissions, and diagnostic procedures. Osogbo Central Hospital will gladly assist you in clarifying your carrier’s requirements, but you ultimately bear the responsibility for ensuring that you meet your insurance company’s guidelines

9. WHAT SHOULD I PACK FOR MY HOSPITAL STAY?

Patients are advised to bring photo identification, toiletries, sleepwear, and personal items. It is also recommended that you bring a complete list of all medications including dosage and frequency with you.

10. WHAT IF I HAVE CONCERN ABOUT MY MEDICAL CARE?

Throughout your hospital stay you are encouraged to discuss your care with your attending physician. You may also request to speak with the Hospital Administrator or Servicom Officer about any concerns that you may have with your treatment plan or hospital stay.
